Once you have scheduled an appointment, please print the forms that pertain to your service. All clients must complete the starred (*) forms and return them prior to the first appointment. The Release of Information and Teacher/Specialist Questionnaire are highly recommended for their importance to understanding an individual’s learning profile.

* Client Questionnaire - Learning Support
This form is completed by a parent/guardian to provide important background information for all services.

* Client Questionnaire - Individual
This form is completed by an adult client to provide important background information for all services.

* Fee Agreement
This form describes the payment policy and your responsibilities for paying for services.

Release of Information
This form allows Sue to receive from, and provide information to, key people such as school staff and other specialists. A separate form must be signed for any specialists not affiliated with a school.

The insights provided by those professionals working with an individual are invaluable in developing a complete picture of that individual’s learning profile.
